Royal Innovation: Cardiac Care

The goal of the Royal Innovation: Cardiac Care program is to improve the quality of life for our residents with congestive heart failure, identify the signs and symptoms of CHF before hospitalization or a more aggressive treatment is required, reduce hospital readmissions, and enable residents and families to make informed decisions regarding care and treatment and be active participants in managing care at home.

We employ CardioNet technology with real time monitoring and analysis and the Zoe Fluid Monitor, identifying CHF up to two weeks before symptoms appear, allowing for a proactive response to fluid changes and avoidance of hospitalization. Discharge planning and education for short term care residents begins on the day of admission. We help schedule follow-up appointments, and make sure the discharge summary is sent to both the resident’s primary physician and cardiologist.
